Once you have downloaded the order form, do the following:

  • Print a hard copy of the order form
  • Manually fill out the form
In the upper right hand box, fill in the Show Name, Show Dates and Show Location
(City, State and Hall)

Please be sure to provide all of the required information at the bottom of the form.

  • Mail the order form with your check to Expo Plant Service, Inc. at the address shown on the order form.
  • You can fax the order to us at (760) 481-7815
